First Time Applicant
 

Adults (age 16 and older)

Please submit:

  • certified, sealed birth certificate, Consular Report of Birth Abroad or Certificate of Naturalization
  • one valid form of photo identification (e.g. foreign passport, driver's license, military ID, etc.)
  • one passport photograph (taken within the last six months); photos can be obtained from a photo booth located in the Consular building of the U.S. Embassy in Rome and the U.S. Consulate General in Milan for 5 euro
  • completed form DS-11
  • a self-addressed stamped envelope with priority mail (Posta Prioritaria) Italian postage. The envelope should be appropriately sized and with correct postage for the return of the passport/s.

All first time applicants must appear in person at the appropriate Consular office.

Children Under 16

Change in Passports for Children:

As of February 1, 2008, the two-parent consent requirement for all passport services was extended to include all children under age 16. Either both parents must be present to provide signatures at the time of passport application, or in the event one parent cannot attend, s/he must send a notarized statement of consent and valid identification. Alternately, the applying parent may present a death certificate or sole custody order for the child.

Please submit:

• certified, sealed birth certificate, Consular Report of Birth Abroad or Certificate of Citizenship
• one valid form of photo identification (e.g. foreign passport, driver's license, military ID, etc.)
one passport photograph (taken within the last six months); photos can be obtained from a photo booth located in the Consular building of the U.S. Embassy in Rome and the U.S. Consulate General in Milan for 5 euro
• completed form DS-11
• a self-addressed stamped envelope with priority mail (Posta Prioritaria) Italian postage. The envelope should be appropriately sized and with correct postage for the return of the passport/s.

Photo Requirements

  • U.S. passport photos are different from Italian passport photos and many photographers in Italy are not familiar with U.S. passport photo specifications. We strongly recommend that you print out the photo requirements for U.S. Passports available on this page of the State Department website and take them with you to the photographer.  Photos can also be obtained from a photo booth located in the Consular building of the U.S. Embassy in Rome and the U.S. Consulate General in Milan for 5 euro.
